Afterward, click the appeal link below to file your appeal directly with SSA. It only takes a couple of minutes to appeal. Do not delay your appeal because you gain nothing by delaying that step. Your case can be time-barred if you do not appeal timely. Also by appealing immediately you prevent many people from filing ahead of you.

Our process after you send the completed questionnaire

Once we receive we receive your completed questionnaire, we will immediately send you the disability application (if you have not filed a claim yet), and other paperwork to sign up to be a client of this firm.

Upon receipt of that completed paperwork, we will review your case, sign and return the Retainer Agreement, help you correct any application mistakes if you have not already filed a disability application, file any application with SSA, and provide you with confirmation of the filing. This service is free of charge. If your disability claim has already been denied, then we will evaluate your case and work on strengthening your case and prepare it for trial before an Administrative Law Judge. Caveat, we reserve the right to refuse to sign the Retainer Agreement for any reason.

SSDI / SSI Questionnaire

For SSDI, complete this questionnaire if: 1) you are currently unable to work due to your disability, 2) your condition will prevent you from working for at least one year, 3) you are currently seeing a medical professional, and 4) you have worked 5 of the last 10 years. -----------------------------------------------------------------------------------------------------------------------For SSI, complete this form if: 1) you are 65 or older OR blind OR disabled, 2) hold US citizenship or legal residency, and 3) you believe you might meet income and resource limits as set forth at https://eligibility.com/ssi. SSI benefits are not limited to adults. Parents of children who are blind or disabled who also have a limited income are encouraged to apply on behalf of their kids.
